Immunization Requirements
Immunizations
If the immunization forms are not presented upon enrollment, students may be allowed 30 calendar days from enrollment to complete the requirements. On the 31st calendar day students without the Georgia Department of Human Resources immunization form will be withdrawn from school. No exceptions to this may be granted without written approval of the superintendent or designee.
The Camden County Health Department or, if you are military connected, the Medical Clinic at Kings Bay Naval Base can provide the immunization forms, or you may go to your private physician.
Measles, Mumps, and Rubella (MMR):
Two doses are now required for all students entering a Georgia school for the first time. For students already in school, the 6thgrade requirement remains in effect.
Notice of Further Immunization Requirements
Hepatitis B Series: All students must have completed the series, or be in the process of completing the Hepatitis B series (3 immunizations) for entry into the school system.
Certificate of Immunization:
I verify that I have been informed that my child’s immunization certificate must be completed within 30 calendar days or he/she will be withdrawn on the 31st day, in accordance with Georgia State law.
VACCINE/IMMUNITY:
All students are required to provide proof of immunity from chicken pox (Varicella vaccine date or year/age child had the chicken pox).
A second dose of the varicella vaccine is now required for all children entering kindergarten, 6th grade or students entering a Georgia school for the first time.
Age Documentation
I verify that I have been informed that all students entering pre-kindergarten, kindergarten and first grade must provide official documentation of age.
Eye, Ear, and Dental (EED) Certificate
This health screening can be performed at the Camden County Health Department or by a private physician. The school nurse can also perform the eye, ear and dental screenings.
I understand that I have been informed that my child’s eye/ear/dental certificate must be completed within 30 calendar days from his/her enrollment.
